Is there a forum host that allows you to...?

Some of these things.

1. Has minimal adverts (so not at top, bottom and after each post.)

2. Allows you to add mods - (if their is one that does this and you tell me i give you best answer).

3. Has great support.

4. Has good domain (so not yoursite.freeforums.com)

5. Has a lot of preinstalled mods such as the Cash mod, Karma and Arcade.

Or, alternativley, is their a free hosting company that you can host a phpbb forum with.
